in identifying and coordinating community-based
resources?

you or someone you know need advocacy and
information about services in your community?
Independent
Living Resources (ILR) provides case management
to assist people with disabilities who are
eligible under the Wisconsin Medical Assistance
program in gaining access to and managing
a full array of services. Due to the demand
for case management services for people with
assets and counties serving people in our
service area, ILR has expanded its case management
program to include Fee for Service case management.
The
goal of case management is to connect people
with the services that support a person's
independence. Case management services include
the coordination of the following services:
- Medical
care
- Socialization
and recreation
- Education
- Vocational
planning
- Housing
resources
- Food
resources
- Transportation
services
- Rehabilitation
services
Case
management services are provided with the
Independent Living philosophy in mind -- consumer
driven and consumer controlled. A consumer
needs to be an active participant in the assessment
of needs, plan development and ongoing service
coordination.
